An amazing and rare opportunity has arrived for a part time Band 6 Community Link Nurse in the NN4 Community Nursing team

We are looking for an experienced registered community nurse to join our dedicated and highly professional team of Community/District nurses working in the Brundall, Acle, Hoveton and Wroxham areas covering the service seven days a week.

This role will offer the opportunity for both clinical and leadership development, whilst working in a caring, friendly and supportive team.

Main duties of the job

You will be responsible for the assessment and treatment of complex patients with a wide variety of health needs of acute or chronic presentation, visiting them within their own homes, care homes, the travelling community and in clinics.  We are looking for a nurse who is committed to delivering high standards of care.  You will supervise Band 5 Community Nurses and other members of the nursing team, and work closely with the therapy staff to assist the patient to achieve maximal function and independence.  In addition, you will support the Clinical Lead Nurse with managerial responsibilities.

You will need effective communication skills and must be prepared to work flexibly and collaboratively to meet the needs of the service.  It is essential to have evidence of professional competencies and have the ability to apply your knowledge and skills to clinical practice.  A sound understanding and knowledge of the current drive to transform community services is an essential requirement.

Detailed job description and main responsibilities

Key Areas of Responsibility

  • To assess and prescribe care packages for patients with long term conditions, the frail and elderly, palliative care and rehabilitation needs for example, to achieve quality of life and independence where possible.
  • To working within the integrated team to facilitate early discharge from hospital.
  • To work within the integrated team to prevent unnecessary admission to hospital.
  • To work with all health care professionals, and statutory/non statutory agencies to provide a seamless integrated service to our services users.
  • To be the direct link to specified medical practices and responsible for managing the associated nursing caseload.
  • To liaise with other link nurses and co-ordinate off duty for Band 6 nurses to ensure service coverage across the locality.
