Job overview

This is an exciting opportunity for a Registered Nurse Learning Disabilities to join Community Services in the Learning Disability Division in Cardiff and the Vale, hosted by Swansea Bay UHB.

As a Registered Nurse you will be leading on the delivery of the health priorities for people with a learning disability with a particular focus on improving people’s access to annual health checks and health action plans via their GP services.

You will have an understanding of the knowledge and skills of the professional activities within a Community Support Team within the field of Learning Disabilities.

You  will be responsible for managing a clinical and case management caseload, and will liaise with both Social & Healthcare professionals to meet client need. You will support less senior members of the team, mentoring and being exemplary role models for high standards of care.

Contribute to the development, maintenance and evaluation of systems and structures, which promote the rights, responsibilities and diversities of people with a learning disability, through effective communication.

The post holders will promote the health and wellbeing of clients by providing specialist nursing assessment and advice and training to clients, carers and other professionals.
