To provide high quality effective physiotherapy assessment and short-term treatment/management programs for adults presenting a range of conditions in Haringey who require a community based rehabilitation service.
To use specialist physiotherapy knowledge and skills to manage own complex caseload, as an autonomous practitioner.
To act as a source of expertise on Falls, orthopaedic and community rehabilitation liaising closely with specialist services; providing advice to patients, carers, all grades of physiotherapists, other health care professionals, consultants, general practitioners and social services.
To support the use of evidence-based practice within the service, incorporating national guidelines e.g. falls guidelines into practice.
To assist with the running of strength and balance classes in the community.

To deliver patient care based on principles of best practice, achievable through direct patient contact or via supervision. To carry out specialist, comprehensive assessments of patients with a complex presentation, using investigative and analytical skills formulating individualized management and treatment plans.
To provide specialized physiotherapy interventions at home or in a community setting as required, collaborating with colleagues in the community and secondary care health professionals to promote continuity of care.
To ensure a timely response to hospital discharges in line with Haringey’s Trust policies.
To work with patients and their carers when appropriate in such a way as to promote their self-management strategies.
To represent the service at the multi-disciplinary team meetings. This will include discussion of patient care, progress and involvement in discharge planning and continued rehabilitation needs.
To keep accurate treatment records, which fulfil legal and statistical requirements, and which are in accordance with HPC/CSP/Trust/departmental standards.
To provide comprehensive progress and discharge reports to GP’s, consultants, patients, members of the multi-disciplinary team and other patient services.
To manage clinical risk within own patient caseload at all times.
To work flexibly to provide cover in other clinical areas when required due to staff shortages.
