The Out of Hours District Nurse Team in East Cheshire NHS Trust are looking for experienced Community Staff Nurses/ Development Band 6 caseload Holders.

Successful candidates will be supported in clinical practice  to work towards the competencies required to become effective Community Caseload Holders and apply for the Specialist Practitioner programme for District Nursing.

We're based at New Alderley House on the MDGH site. Our service covers the whole of the East Cheshire area and provides a crisis response to community patients. We work closely with the Out of Hours GP service, NWAS and the Hospice at Home Team.

This is an exciting opportunity to join our innovative team to drive forward the expansion and modernisation of community nursing service with commitment to deliver the NHS Long Term Plan. Travel is an essential part of this role. Please demonstrate on your application how you would meet this requirement.

Candidates who don't currently have the V150 prescribing qualification may be considered and are encouraged to apply.

Hours per week: 15 hours per week

Main duties of the job

To provide holistic, high quality, safe and skilled health and social care / support to patients, carers and their families

To provide a timely crisis response service for unpredictable needs across the East Cheshire area

To work closely with members of the wider Out of Hours services to provide rapid and planned expert professional assessment and treatment interventions for patients in their own homes or other community settings, putting patients at the centre of their care and enabling  patients to maximise their independence, health and wellbeing and remain in their own homes for as long as possible

To be responsible for coordinating and triaging referrals into the service and support the assessment of care needs, the development, implementation and evaluation of care programmes.

To provide leadership and support across the Out of Hours District Nurse team demonstrating emotional intelligence to recognise pressures of self and team
