About the role: Under supervision of Registered Nurses, you will deliver care coordination services to young people and their families, enhancing access and collaboration throughout their care journey in CAMHS.

Main duties of the job

Reporting into the Clinical Nurse Specialist, your responsibilities will include:

• Provide care coordination to young people and their families, ensuring the smooth access and communication across services.
• Collaborate with multidisciplinary professionals and external agencies to support the holistic care of young people.
• Be the lead contact for families, responding to telephone enquiries and providing support regarding patient care.
• Facilitate communication and information sharing between the multi-agency network, incorporating safeguarding, education, and transition issues into the therapeutic process.

Please note that Sheffield Children's uplift pay to meet the Real Living Wage, which is £12.60 per hour.
