Job overview

This is an exciting and innovative opportunity for an individual who has a keen interest in digital health and technology.  You will be responsible for supporting flow and capacity across the surgical division by enabling suitable patients to either have an earlier discharge or to prevent an inpatient admission, by providing acute hospital level care within their own homes.

Main duties of the job

Day to day management of cohorts of patients who are being cared for remotely; using digital technology to monitor and provide elements of their care. Working within local and national guidelines to provide assessment, treatment plans, escalations and evaluations.

Part of the overarching Trust Virtual Ward / Hospital@Home service, working within a multi professional team, working closely with acute and community practitioners.

Detailed job description and main responsibilities

  • holistic assessment, planning, implementation and evaluation of evidence-based care for patients referred to the virtual ward / hospital@home service
  • use remote monitoring and technology as part of care delivery
  • manage clinical events and exercise clinical judgement to manage risk appropriately
  • contribute to the development of the service and quality of care
  • use professional judgement on when to escalate / seek help as appropriate
  • role model the values of the organisation, service and wider healthcare system
  • participate and contribute to new models of care and service redesign
  • identify education and development needs of self and others
  • demonstrate a commitment to research-based practice
