Job overview

At Sandwell and West Birmingham NHS trust we are very proud of our specialist palliative care and End of life care Co-ordination Hub. The CQC awarded us excellent for our service in 2017. The team consists of specialist Palliative consultants, specialist palliative care nurses for the Acute and community, end of life care facilitators, Urgent response team, day Hospice and Macmillan therapists.

The service is a 24/7 service and provides care to patients who are in hospital and the community in Sandwell and West Birmingham. You will need to have good clinical knowledge around end of life care, complex symptom control and advanced communication.

Please be aware that this is role is for a specialist palliative care nurse across Acute and Community services including virtual ward and will mean base is either at MMUH or Oldbury Council House.

Main duties of the job

We are recruiting a Band 7 Specialist palliative care nurse. You must be an experienced registered nurse and have significant previous experience in the specialty or a related specialty. You must be willing to work and cover on call over 7 days. You will be given the opportunity to  rotate between between acute, community and Living Well Service with support and training.

We also encourage our Specialist Palliative Care team to take part in education both within the team and across the trust and will be expected to take on project work to improve service delivery.

When visiting patients with complex palliative care needs, the post holder must recognise that he/she is not the primary carer, but must seek to equip the primary carer with the required skills and knowledge, so that the highest standards of care can be made available to the maximum number of patients.

Demonstrate advanced practice in holistic assessment, planning, implementation and evaluation of patient care within their own speciality. This will necessitate high levels of autonomy and advocacy and include receiving direct referrals and ensuring the delivery of appropriate care for a caseload of patients.
