Job overview

The Cancer CNS provides support to patients who have been referred in to the London Sarcoma Service, all the way from investigations, diagnosis and treatment. The CNS is the Keyworker, providing contact and information. The CNS will be present in Outpatients,  will visit patients on the wards when having surgery, and liaises closely with other professionals to ensure seamless co-ordination of treatment pathways when patients are having chemotherapy and/or radiotherapy in other Trusts . They are  a core member of the MDT (multidisciplinary team), and present and record outcomes. They undertake Holistic Needs Assessments (HNA's). They provide teaching to other members of the nursing staff.

Provide information and act as support for patients undergoing investigations for a suspected cancer. They are present in outpatients when patients are receiving a cancer diagnosis. They act as the patients Keyworker. They liaise with other Trusts to ensure seamless treatment pathways when patients are undergoing chemotherapy and/or radiotherapy pre or post surgery. The visit patients on the ward having surgery. They take part in the weekly MDT's, both presenting patient information and recording MDT outcomes. Complete HNA's with patients and signpost to other services as appropriate. Provide teaching to other nursing groups.
