We are seeking two  dynamic and compassionate Clinical Coordinators to join our team in a newly created role supporting children and young people with Special Educational Needs and Disabilities (SEND).  This  post is part of our commitment to delivering on SEND  improvement priorities.

As Clinical Coordinator, you will play a key role in ensuring timely, high-quality clinical input into Education, Health and Care Plans (EHCPs), improving multi-agency collaboration, and enhancing service coordination across health pathways. Main duties of the job

Coordinate Clinical Input for SEND Pathways Ensure timely and high-quality clinical contributions to Education, Health and Care Plans (EHCPs), in line with statutory requirements.

Facilitate Multi-Agency Collaboration Act as a key liaison between health services, education, and social care to support integrated planning and delivery for children and young people with SEND.

Support SEND Inspection Improvement Actions Deliver on specific improvement priorities identified through SEND inspections, ensuring compliance and service transformation.

Provide Oversight and Guidance Offer expert advice to frontline teams, helping to manage complex cases and reduce variation in care.

Monitor Performance and Quality Track service delivery against key performance indicators, contributing to audit, evaluation, and continuous improvement.

Engage with Families and Professionals Support effective communication with families, carers, and professionals to ensure coordinated care and positive outcomes.
