An exciting opportunity has arisen to join the Children's Community Nursing team here at Stockport for a 9 month fixed term or Secondment covering maternity leave

You will be responsible for delivering care to children with acute and chronic health needs within their own homes across 7 days.

You should be experienced in working with children who have acute health needs and an understanding of children with complex health care needs.

You must be able to work flexible between the hours of 8am till 9pm 7 days a week and be able to support an overnight on call service for children who are end of life.

You will be at the forefront of assessing the child, planning, and implementing care and evaluating success.

Safeguarding of children is paramount, and you will be responsible for promoting this by implementing the Trusts policies and procedures and acting promptly on concerns, communicating effectively and sharing information appropriately.

Main duties of the job

The post holder will work as an experienced and autonomous practitioner within the Children’s Community Nursing Service, providing high-quality, evidence-based nursing care to children and young people with a range of acute, chronic and complex health needs. The role requires clinical expertise, leadership responsibilities, and contribution to service delivery, development and evaluation in line with Trust objectives and national standards.

Communicate effectively with children, young people and families, adapting communication style to meet developmental, cultural and emotional needs.

Provide complex and sensitive clinical information in a clear and empathetic manner.

Communicate effectively with members of the multidisciplinary and multi-agency team, including health, education and social care professionals.

  • Manage and prioritise a daily caseload, balancing competing demands and changing priorities.
  • Plan, coordinate and organise care delivery across community settings to ensure continuity of care.
  • Organise own workload and contribute to team workload planning.
  • Participate in service planning activities to support effective and efficient service delivery.
  • Use Trust resources, equipment and supplies efficiently and responsibly.
  • Ensure safe use, storage and maintenance of clinical equipment.
  • Identify and report issues relating to equipment, stock or resource availability.
  • Contribute to cost-effective care delivery through appropriate use of resources
  • Implement Trust, local and national policies, procedures and clinical guidelines.
  • Contribute to the review, development and implementation of policies, pathways and protocols.
  • Participate in service development and quality improvement initiatives.
  • Support the implementation of change within the service to improve patient outcomes and experience.
  • Supervise, mentor and support junior nursing staff, support workers and students.
  • Act as a role model, promoting professional standards, equality, diversity and inclusion.
  • Contribute to recruitment, induction and retention of staff where appropriate.
  • Support the education and development of students and other staff.
  • Deliver training, teaching and health education to children, families and carers.
  • Participate in the delivery of in-service training and sharing of best practice.
  • Maintain own professional development and mandatory training requirements.
  • Maintain accurate, contemporaneous and confidential patient records using electronic systems.
  • Ensure compliance with information governance and data protection legislation.
  • Collect and input data required for service monitoring, reporting and evaluation.
  • Use information and data to inform clinical practice and service improvement.
  • Participate in audit, service evaluation and quality improvement activities.
  • Support implementation of research findings and evidence-based practice.
  • Use audit outcomes to inform changes in practice and improve care delivery.
  • Maintain awareness of current research and professional developments relevant to children’s community nursing.
  • Undertake clinical procedures requiring precision, dexterity and accuracy.
  • Travel independently across a range of community settings, including homes, schools and clinics.
  • Manage the physical demands of community nursing, including moving and handling equipment.
  • Maintain concentration and emotional resilience when caring for children with complex or distressing needs.
