Job overview

The Community Paediatric Dietitian will be a member of the Paediatric Dietetics Community Team and will use assessment and diagnostic skills to care for patients and advise primary care health professionals on future care. The post holder will be an autonomous practitioner, collaborating with colleagues in primary care, community, and hospital settings as appropriate.

Main duties of the job

  • Provide specialist paediatric dietetic assessments and treatment interventions to children and families within health centres, children’s centres and other locations as appropriate, within Merton.
  • Liaise with other agencies and healthcare professionals and maintain accurate clinical records as per the clinical record keeping policy.
  • Build strong networks with GP’s, NHS Health Care Teams, Health visitors, School nurses, and other referral agencies and independent contractors to facilitate appropriate referrals.
  • Work with the clinical lead and medicines management team to implement paediatric appropriate prescribing initiatives across CLCH.
  • Initiate and participate in clinical governance activities/ audit within the Integrated Complex Needs Team.
  • Ensure community health care teams and non-NHS organisations have access to up to date nutritional information through development of education and training sessions.
  • Act as a source of expertise to members of Integrated Complex Needs Team and other Health Professionals within CLCH.
  • To be able to read, action and implement dashboard/ service data to affect timely patient intervention under the direction of the B7 paediatric dietician.
