An exciting opportunity has become available to join our Inpatient Rehabilitation Multi-Disciplinary Team at Meadow Court, in the role of our CHC Clinical Nurse Assessor and Discharge Facilitator.

We are seeking a motivated and forward thinking registered qualified nurse to support the flow of patients through our inpatient rehabilitation units, whilst ensuring all patients discharged onto the D2A nursing home pathway are assessed against the criteria for NHS Continuing Care.

The post holder will be responsible for the effective coordination of patient discharges throughout the inpatient units across Meadow Court.

The successful candidate will need to be able to demonstrate knowledge, skills and experience of person-centred assessments and discharge facilitation. The role will involve working within a multidisciplinary team and a multi-agency environment to support patients' discharges from the wards, the assessment and review process for NHS Continuing Healthcare (CHC) and NHS Funded Nursing Care (FNC).

Experience undertaking mental capacity and best interests' assessments is advantageous and an understanding of deprivation of liberty safeguards and the Court of Protection would be beneficial to the role. You will also need to be knowledgeable in report writing to ensure you can confidently complete a health needs assessment, personal care and support plan and the CHC decision Support Tool.

Main duties of the job

You will need to be competent in using Microsoft Word, Excel and Outlook and also be confident in learning new systems for recording and report writing.

The successful applicant will be required to work both autonomously and collaboratively in a team and be able to adapt to a rapidly changing environment.
