Clinical Change Facilitator

  • Are you a registered healthcare professional who is passionate about improving patient care and helping positive change take root in your clinical area?
  • Do you enjoy using digital tools and recognise the difference they can make for staff, services, and patient experience?
  • Are you organised, enthusiastic, motivated, and a confident communicator who is ready for a rewarding new challenge in your nursing career?

This is a meaningful opportunity to join the Clinical Informatics team, where you will play an important part in helping colleagues make the most of digital tools and supporting improvements that have a real impact on patient care.

In this role, you will help drive the successful adoption of new initiatives and technologies, working alongside front line staff to support lasting improvement. You will bring your clinical knowledge, skills, and ability to engage others to identify where changes to practice, tools, operational processes, or skills can strengthen the quality of care and improve outcomes for patients.

We are looking for someone who can demonstrate the Trust values and bring the experience, commitment, and qualities set out in the job description and person specification.

Main duties of the job

This role will involve the application of clinical knowledge to assess the adequacy of current ‘systems’ in clinical areas, meaning are the tools, associated processes, and skill set working together for effective patient care. The role will determine if a change to practice, tools, operational processes or skill set is required to deliver an improvement.

Detailed job description and main responsibilities

In line with strategic direction of the Digital Data and Technology (DDaT) Department, the postholder will be deployed to clinical areas to drive improvement by supporting staff through service changes, digital implementations, or new ways of working. They bridge the gap between digital teams and clinical staff, organizing workshops, gathering feedback, and facilitating training to ensure safe, patient-centred care.
