An  exciting  opportunity has arisen for  motivated individual to work within the Community Nursing Division as a Community Nursing Caseload Manager,  within the Wallasey Locality working with the Liscard Community Nursing Team in Wallasey.

Main duties of the job

30 hours per week.  Hours and shift patterns to be discussed at interview.

This post will be responsible for managing planned programmes of care for patients in domiciliary and clinic settings.

A main part of the role of a Caseload Manager is to assess, plan, implement and evaluate nursing care to patients across the community/primary care interface in the most efficient and clinically effective way, in co-operation with other members of the health and social care teams, and voluntary and statutory agencies.
