Job overview

We are excited to announce that we are expanding our Advanced Nurse Practitioner Service within Cardiac services here at Imperial Healthcare NHS Trust.

If you are an experienced Band 7 ANP who holds a Physical Assessment and Non-Medical Prescribing qualification we encourage you to apply. The successful candidates will be expected to perform within the four pillars of advanced practice.

The Advanced Practitioner team work long days, (no nights) and provide a seven-day service across our acute inpatient areas including our busy Heart Assessment Centre.

This role involves liaising with other referring hospitals for repatriation post treatment or intervention and play a key part in safe discharge home following OT/Physio and Discharge Team input.

Our ANP’s are key to patient flow into and out of the hospital and across key areas and are part of the multidisciplinary daily ward rounds and subsequent board rounds.

The ANP’s are responsible for timely patient movement from Trust acute wards to cardiac wards during out-of-hours and weekends, offloading patients faster and supporting emergency and elective admissions.

Main duties of the job

Advanced clinical practice embodies the ability to manage clinical care in partnership with patients, families and carers.  It includes the analysis and synthesis of complex problems across a range of settings, enabling innovative solutions to enhance people’s experience and improve outcomes.

  • Review and prescribe medication in a timely manner ensuring it is within own level of knowledge and competence and within the scope of the Trusts Policy for non-medical prescribing qualification formulating of discharge documentation including medications and onward follow up care.
  • To review urgent patient referrals for cardiac patients within the trust and coordinate the urgent referrals from other NHS organisations. Communicating with the Senior Registers and Pre assessment team to manage and plan the patient in preparation for surgery.
