Within a multi-disciplinary environment, assist in the delivery of a comprehensive programme of secondary/primary prevention of cardiovascular disease and other cardiac diagnoses. The programme will be delivered within the Wirral Heart Centres across the St Catherine’s Health Centre, Victoria Central Health Centre and West Kirby Marine Lake sites. The post will require performing ECGs, taking basic measurements such as height, weight and blood pressure. Excellent communication skills and an ability to work in a team and to direction is essential. Using initiative and having excellent organisation skills is key. Team work is a major feature of this role and the ability to work and communicate with a diverse and multidisciplinary team.

Detailed job description and main responsibilities
4.1 Clinical Responsibilities
- Undertake safe storage, and disposal of medicinal products in line with Trust policies.
- Collaborating effectively with other members of the multidisciplinary team or other agencies involved in the person’s care.
Recognise and apply knowledge of commonly encountered mental, physical, behavioural and cognitive health conditions when delivering care. Additionally demonstrate and apply an understanding of how and when to escalate to the Caseload Manager.
support, supervise and act as a role model to nursing associate students, health care support workers and those new to care roles, review the quality of the care they provide, promoting reflection and providing constructive feedback.
Work in partnership with people, to encourage shared decision making, in order to support individuals, their families and carers to manage their own care when appropriate. Whilst minimising risk for both people and staff, making use of all available resources.
Promotes and applies the aims and principles of health promotion, protection and improvement and the prevention of ill health when engaging with people
Protect health through understanding and applying the principles of infection prevention and control, including communicable disease surveillance and antimicrobial stewardship and resistance.
Assist in the deployment and organisation of the intermediate phase of cardiac rehabilitation to include educational talks, exercise and relaxation classes
Ensure adequate levels of supervision of all rehabilitation sessions in line with service policies and procedures and according to the patient’s care plan
As part of promoting a safety culture recognise and report via Datix, any situations, behaviours or errors that could result in poor care outcomes
Is involved in audits and distributes and encourages completion of patient experience questionnaires.
Maintains good working relationships with colleagues and other agencies. Understand the demands of professional practice and demonstrate how to recognise signs of vulnerability in themselves or their colleagues and the action required to minimise risks to health

Recognise and respond to adverse or abnormal reactions to medications, and when and how to escalate any concerns to Caseload Manager and Clinical Team Leader 10.
