Job summary

Main duties of the job

Working under the guidance of the Lead Nurse and Clinical Nurse Specialists, you will help patients navigate the complexities of their treatment and follow-up care, ensuring they move smoothly onto their Risk Stratified Follow Up (SFU) pathway.

You will be instrumental in co-ordinating appointments, assessments and investigations, supporting holistic needs assessments, promoting self-management, and ensuring every patient experience reflects dignity, respect and the highest standards of safety. This is a rewarding role for someone who is organised, empathetic and committed to delivering personalised, patient centred care.
