An exciting opportunity has arisen for two secondment post for Band 7 Bed manager covering two maternity posts until April 27.
If you want to work in a high performing team, organised and enjoy fostering good relationships, we have an exciting opportunity to join the Patient Flow Team operationally based across our two inpatient units.
The post holder will be the central point for all referrals for inpatient admissions and will be working within an integrated team alongside our Inpatient Flow team and bed management team to ensure effective patient flow which includes reducing the length of inpatient stay. They will also be responsible for ensuring comprehensive and coordinated discharge planning processes are in place including working with patients with any barriers to their discharge pathway and who are Clinically Ready for Discharge (CRFD). The post holder will also hold a key role in supporting the Acute Trust with efficient transfer once a Decision to Admit has been made to minimise any breaches.
The post holder will work closely with the Patient Flow Service Manager and Patient Flow Lead Practitioner, and deputise where required, to manage the inpatient bed provision across the Adult Acute Inpatient Services in Hounslow and Hammersmith & Fulham including PICU and Older Adults Inpatient Services. The post holder will be expected to work closely alongside the Senior Management Team, Inpatient Leadership Teams, CATT teams, Community Teams and key Trust and external professionals to ensure effective patient flow in maintained.
We are looking for someone who is very confident in managing complex and challenging decisions with a robust clinical risk assessment and management experience and knowledge. You will be expected to have a strong team work ethic, excellent communication skills and sense of accountability accompanied by an ability to work under considerable pressure at times. In return you will be supported by robust clinical leadership and we will provide support and training for this role and on-going supervision and the development of your skills.
