Central London Community Healthcare NHS Trust (CLCH) is seeking a proactive and organised Band 4 PCN Co-ordinator (Bank role) to support the delivery of high-quality, patient-centred care across Primary Care Network (PCN) / Neighbourhood services within the Community Diabetes Service across Brent and Harrow.

This is a temporary role for 2–3 months, working Monday to Friday, 09:00–17:00 (full-time), with the potential for extension depending on service need.

Role Overview

The post holder will play a key role in coordinating PCN / Neighbourhood activity, with a particular focus on supporting the Diabetes PCN Multidisciplinary Team (MDT). You will work closely with GP practices, neighbourhood teams, and the Community Diabetes Specialist Service to support integrated, patient-centred care delivery.

Main duties of the job

  • Coordinate the day-to-day delivery of the Diabetes PCN MDT / Neighbourhood services
  • Support planning and implementation of new services in line with MDT priorities
  • Ensure effective communication and coordination between GP practices and partner organisations
  • Manage appointments, referrals, and service activity using electronic systems
  • Organise and support MDT meetings and service-related discussions
  • Monitor service performance, contribute to data collection, reporting, and continuous improvement
  • Track progress against objectives and support reporting of outcomes
  • Work collaboratively with GPs, practice staff, multidisciplinary teams, and external partners
  • Promote strong integrated working across the PCN to support efficient service delivery
