Job overview

To provide a high-quality specialist Clinical Pharmacy Service across a range of inpatient settings in the Community Hospitals (Rehabilitation Units, Interim Care Beds, Palliative Care, Discharge to Assess wards). To provide professional expertise, advice, support and training to health care professionals and patients within the Trust.

This role is required to cover Bedded Services in Hertfordshire and North London

Sites include:

St Peters, Hemel Hempstead Hospital. HP2 4AD

Oakmere South and North, Potters Bar Community Hospital. EN6 2RY

Midway and Holywell ward, Langley House. WD25 9FG

Main duties of the job

  • To provide and develop a high-quality specialist Clinical Pharmacy Service across a range of inpatient settings in the Community Hospitals (Rehabilitation Units, Interim Care Beds, Palliative Care, Discharge to Assess ward) covered by the Bedded Services team supported by the Lead Pharmacist for Hertfordshire and Lead Pharmacist for Bedded Services.
  • To provide a patient focused pharmacy service to patients and staff including medicines reconciliation and full clinical screening of prescriptions to ensure safe and cost-effective use of evidence based and rational use of medicines, in line with local policies and national guidance ensuring medicines are optimised and risks to patients are minimized e.g. NICE, NHSE/I, PHE
  • Facilitate changes to the patients’ own use of medicines to improve safety, adherence and empower as much independent use of medicines as possible and appropriate.
  • To support and implement efficient medicines management initiatives to reduce drug waste and drug spend and to support patient flow and facilitate timely discharges
  • To participate in ward rounds and MDTs where appropriate to ensure a multidisciplinary and integrated approach to medicines management, with a view to ensuring continuity of pharmaceutical care for patients in different healthcare settings.
  • To optimise medications by undertaking Structured Medication Reviews and provide counselling to all patients
