Job overview

We are currently seeking an experienced Community Mental Health Nurse, at Band 6 to work via a Bank Contract, based in Welwyn for our Eating Disorders Service. You will be joining our Temporary Staffing Team within HPFT - Hertfordshire Partnership University NHS Foundation Trust.

The post holder will work in close collaboration with other professionals, including the wider multi – disciplinary Community Eating Disorder Services team, in assessing service user’s needs, devising care plans and evaluating outcomes that promote service user empowerment and recovery. The post holder, with the Multi-Disciplinary Team (MDT), will provide high quality, individualised and evidence-based eating disorder care and treatment, which addresses the needs of the service users and their families and meets individual specialist health needs.

You will be expected to attend meetings or training courses across HPFT in Hertfordshire and be expected to drive their own vehicle regularly between bases and must cope with the physical demands of frequent travel across the Trust and further afield as required.

Main duties of the job

The Community Mental Health Nurse will:

  • Be responsible for the assessment, planning, implementation and evaluation of high-quality specialist care for people with moderate to severe eating disorders.
  • Undertake training in phlebotomy to monitor the physical health of people with severe eating disorders under the guidance of the Consultant Psychiatrist
  • Manage high levels of risk in the community in partnership with the consultant psychiatrists and senior support workers, promoting service user recovery as well as safety whenever possible
  • Be responsible for liaising with the GP practices in the quadrant for which the nurse is responsible.
  • Be required to teach other nursing and non-nursing personnel.
  • Be a role model through their own good practice.
  • Be responsible for maintaining clinical standards, and team systems under the direction of the Team Leader and Professional Lead.
  • Assist the team in maintaining effective team working processes, including outcome measures.
  • Be expected to deputise for the Team Leader in their absence.

Detailed job description and main responsibilities

  • Function as a lead professional / care co-ordinator to a caseload of people with moderate to severe eating disorders and associated complex health needs as allocated, taking responsibility for formulation of appropriate care plans
  • Co-ordinate all co-workers assigned to his\her service user group and will ensure they are fully conversant with all aspects of the service user’s care and treatment.
  • Identify the individual care needs of allocated service users, plan, implement and evaluate all care programmes in conjunction with the service’s nursing team and the MDT.
  • Provide effective professional support of support workers within the service.
  • Provide mentorship and support for student nurses on placement in the service
  • Provide nursing care in a person-centred way that promotes and facilitates the positive health and well-being of people with eating disorders
  • Maintain close liaison with all department\staff involved in a service user’s care and report on progress and effectiveness to the MDT.
  • Lead and/or participate in reviews within community and inpatient settings / services and will prepare reports on service users as required.
  • Be aware of all policy documents and legal requirements pertinent to the post and ensure that these are adhered to.
  • Demonstrate and serve as an example for all clinical activities to appropriate grades of staff
  • Be fully conversant and ensure compliance with NMC codes of conduct and practice, relevant legislation and policies and procedures.
