As a Memory Service Nurse in North Somerset, you will participate as a full member of the multi-disciplinary Memory Service team.
You will be responsible for coordinating the service user’s journey through their memory assessment through to diagnosis.
You will play a key role in preparing the service user and carers for the diagnostic process and in seeking consent from the service user regarding further assessment as appropriate.
You are expected to present your cases to the MDT each week.
You will be expected to work across several locations, including the service user’s own home if indicated.
Main duties of the job
- To provide initial specialist assessment for individuals experiencing problems with their memory and cognition.
- Holding a caseload of service users whilst the process of their assessment and diagnosis progresses.
- To liaise and participate with other members of the multidisciplinary team, family members, carers, Primary Care and other agencies involved with individual service users to formulate diagnoses.

- To communicate diagnosis to individual service users and those in their care network, both in person and in writing.
- Signpost or make referrals for service users and carers to other services as appropriate.
- To provide support and advice to Carers through individual work or signpost/refer onwards where indicated.
- To be able to facilitate/lead in post diagnostic interventions offered by the service, including ‘Living Well with Dementia’ groups and or ‘Memory Matters’.
- To contribute to integrated governance processes, including the continuing development, improvement and monitoring of service provision and performance, and monitoring standards of care, through data collection and audit processes.
- To work in partnership with other local statutory and voluntary organisations in the provision of a range of interventions and support for service users and carers.
- To provide support, advice and training as appropriate to other services and agencies involved in the provision of services to people with memory problems.
- To be responsible for the supervision of identified staff and provision of training for students of various disciplines as appropriate.
