Job overview

We are currently seeking a highly motivated and enthusiastic Mental Health Nurse to join our team.

Bristol Community Rehabilitation Service includes a multi disciplinary community team and a recovery focused 10 bed accommodation service. We work with people with complex mental health needs who could benefit from intensive support and therapeutic interventions. Our service is focused on providing effective interventions to support people in achieving their recovery goals. The service has a particular focus on supporting the inpatient rehabilitation pathway including people who have been in out of area rehabilitation placements.

The team includes a wide range of skilled and highly motivated staff including Peer Workers, Recovery Navigators, Mental Health Nurses, Occupational Therapists, a Social Worker, a Consultant Psychiatrist, and Clinical and Counselling Psychologists.

Main duties of the job

You will carry out specialist rehabilitation assessments, provide rehabilitation focused interventions including psychosocial interventions, medication management and administration, Key Working, and working with carers.

You will also provide  Recovery Navigators in the team.

The successful candidate will work primarily in the community team.

Detailed job description and main responsibilities

  • To undertake the full range of activities required to deliver an ongoing comprehensive mental health service.  2. To facilitate the delivery of Personalised Care and Support Planning in line with the Your Team/your Conversation-Your Plan” Policy and guidance.
  • To record risk incidents and to collaboratively create and update Safety Assessments and Safety Plans  4. To provide personalised interventions to support people with Mental Health difficulties  5. To sometimes act as a Keyworker for people to whom you are delivering interventions. A keyworker is someone who is the key point of contact for people and their families and other teams and services. Although it is everyone’s responsibility, Keyworkers take a lead role in making  sure that everyone who is working with the person understands the plan and is working together as a team. Keyworkers take a lead responsibility for having Support Conversations and making sure that the Personal Wellbeing Plan makes sense.
