Are you interested in being part of a responsive, innovative and dynamic service, delivering patient care in a supportive and dedicated team?

We are looking for a Band 5 Physiotherapist to join our High Peak Community Therapy Service, working across our bedded care units. We are increasing our bedded care offer, and need to provide an increase in access to physiotherapy. The two bedded care facilities are Fenton Ward (Cavendish Community Hospital) and Thomas Fields  (Community Supported Bedded Unit, Derbyshire County Council) The therapy team is comprised of Physiotherapists, Occupational Therapists and Support Workers who work closely with the wider multi-disciplinary team to provide rehabilitation and reablement supporting patients to return home or providing in-put to those needing step-up care.

This post is an excellent opportunity to develop your skills and knowledge working within supportive environments with access to a preceptorship programme.

Once competent, you will be expected to participate in Rapid Response duties. Rapid Response service operates between 8am and 6:30pm, with shifts of 8am- 4pm and 10:30am and 6:30pm.  You will also be expected to participate in the weekend duties.

Main duties of the job

To undertake assessments of adults who are referred to the therapy service and to be able to deliver safe and effective physiotherapy intervention.

To be an integral member of the Integrated Community Team (ICT) delivering high quality, multi-disciplinary health and wellbeing interventions.

To be an active member of the wider Physiotherapy workforce of Derbyshire Community Health Services NHS Foundation Trust and willing to work wherever the service requires.

You will be responsible for maintaining competency to practice through maintenance of professional registration, Continuous Professional Development activities and main a portfolio which reflects personal and professional development and evidence of learning beyond registration.

You will need to have a BSc/Diploma in physiotherapy and be registered with the HCPC.

As a clinical employee it will be your responsibility to ensure that your behaviour and values reflect those set out in the DCHS Way
