Main duties of the job

The main duties of the role include:

  • To implement identified care plans-direct delivery of care to our residents, under direction/supervision of Nurse in charge as part of their daily care needs
  • Report observations of residents to Nurse in charge, to ensure appropriate review and evaluation of care plans take place. Monitor and report changes in patients’ physical and emotional well- being to the Registered Nurse in Charge/ nursing team in order to appropriately escalate concerns, all working towards working together as a multi- disciplinary team.
  • To maintain and promote person centred care in all aspects of service delivery ensuring resident/relative engagement and choice is part of this. Providing high quality, person- centred care, to those with complex dementia, older adult physical health and mental health conditions.
  • To promote independence, choice, dignity, and respect throughout all hands-on care delivery. Supporting patients with personal care, mobility and activities of daily living by assisting with personal cares and activities of daily living, depending on the level of need assessed in the moment and what is documented within individualised care plans and risk assessments
  • As part of the team, you will be responsible for maintaining the highest standards of care, following individual care plans. Maintain accurate and timely records of care in line with Trust Policy and Procedure

Detailed job description and main responsibilities

A few examples of Healthcare support worker duties expected are as follows:

  • Personal care delivery, including, washing, personal hygiene, dressing and feeding. Ensuring physical and emotional needs of residents are met.
  • Engaging with activities, in and out of Birch Avenue to ensure emotional wellbeing is maintained for our residents, including involving their families and loved ones.
  • Developing positive relationships with resident, relatives, and staff, maintaining dignity and respect of all.
  • To document all care delivered as part of shift and work alongside colleagues as key worker for identified residents.
  • To engage with and support relatives of relatives who are allocated within keyworker premise.
  • To escort residents to areas within and out of the home, for, but not limited to social activities, healthcare appointments or transfers.
  • To engage residents in meaningful activities as part of the day- to- day role.
  • To assist and, report to, Nurse in charge, to ensure thorough and appropriate review and formulation of care plans is completed in a timely manner.
  • Working flexibly to meet service requirements, to trust policy and procedures and maintain a safe environment in accordance with Health and Safety legislation.
  • To share identified skills with colleagues to ensure our residents receive the best care possible.
  • Take responsibility for personal and professional development, participation in supervisions and appraisals as per policy, and participate in agreed training and development sessions relevant to role.
  • To take an active part in agreed learning, mandatory training, continued professional development, handovers, GP round, multi- disciplinary team discussion, Safety Huddles, Supervisions and Personal/ Professional Development Reviews
  • To provide meaningful activities to provide well rounded emotional well-being and empowerment, and quality of life to the patients (residents)
