we are delighted in over a Band 4 permanent position. Based at Billingham Health Centre working within the Community Integrated Assessment Team. This is an exciting opportunity to join the Community Integrated Assessment Team working in line with the New NHS 10 Year Plan. Working alongside Multifidisciplinary teams, in preventing hospital admissions, supporting safe discharges from hospital, provide a rapid response and ongoing assessment for patients either at home or in a community setting.
Undertaking clinical tasks within competencies, supporting planning and carry out patient previews, including liaising with families, carers, social workers, nurses and Gp's. Supporting the co-ordination of referral from out Integrated Single Point of Access and other community services that would also be identified as appropriate for those patients under the care of the Community Integrated Assessment Team.
This role support the Band 5's and Band 3's with a future potential for those seeking further opportunities in future career development.

Actively and effectively, communicate with the patient, carers, health, social care, the voluntary and private sector agencies as appropriate. Be responsible for ensuring problems identified following a direct or indirect patient contact are addressed in the outlined treatment plan. Be able to work towards successful completion of NEWs and clinical skills competencies which will include being able to refer identified complications and “red flags” appropriately to registered staff if a patient is not progressing as expected. Be responsible for ensuring all problems identified following assessment are addressed in a treatment plan and that a patient is discharged appropriately from the caseload when all goals are met and referred onto relevant other agencies. Ensure those patients with less complex needs receive the appropriate support to be safe in their chosen environment, including the assessment for and the provision of equipment as highlighted in the band 4 competency framework.
Be responsible for completing the initial subjective/objective assessments on triaged noncomplex patients and complete relevant documentation following assessments to demonstrate clinical reasoning. Support registered staff in the assessment and intervention of more complex patients and be responsible for the continuing assessment following a baseline assessment performed by registered staff. Undertake more complex interventions under the direction of the registered Therapist. Work closely and liaise with all members of the Multi-disciplinary Team ensuring a seamless pathway of care. Effectively develop and implement a prescribed therapy plan including mobility/functional interventions to enable people to be safely rehabilitated in their home environment or residential rehabilitation unit. Participate in seven day and / or extended hours working if required to do so. Deal sensitively with patients who have high levels of anxiety and aggression caused by pain, dementia or limited mobility in frequently distressing or emotional circumstances, providing care for distressed patients clients or clients with challenging behaviour. Able to prescribe equipment and walking aids to enable patient’s functional safety and mobility. Promote dignity and privacy to all patients. Ensure responsive and timely interventions to people in their own homes or needing support to return home or to a safe environment. Work in unpredictable community settings where working conditions may be unpleasant
