We are currently recruiting two highly motivated and compassionate Assistant Practitioners to join our dynamic and friendly Adult Community Mental Health Team (CMHT) in South Norfolk. This is a fast-paced and rewarding environment where your skills and dedication will be truly valued.

Main duties of the job

Working alongside our Registered Nurses, Social Workers and Occupational Therapists, you will support with the day to day activities and provide support to service users throughout their recovery journey. You will hold and manage an identified caseload where appropriate; take part in and contribute to ongoing review of risk assessments, recognising and responding to any changes; attend and contribute to MDT reviews; and liaise with family and carers.

Working within an experienced team, you will be supported with regular supervision and opportunities to work in partnership with health and social care in the wider multi-disciplinary service.

This role requires the ability to travel to various locations to meet service needs. Access to a vehicle for work purposes is preferred, as some office sites and client care bases may not be easily reached by public transport.
