Job summary

Main duties of the job

The post holder will carry out planned and unplanned health (nursing) interventions as designated by the lead clinician, and will work both supervised and unsupervised in patients own homes, clinic based wound care hubs, residential, nursing and other intermediate care facilities, reporting back patient progress and informing the lead clinician of any problems

The Assistant Practitioner will provide advanced support to the registered nurse and will work within the defined Trust policies, procedures and competencies and treatment plans for patients creating safe, caring environment that provides privacy, dignity and respect for the patient.
