We are seeking a compassionate, motivated, and proactive individual to join our team, delivering high-quality, person-centred care and support to service users and their families.
This diverse and rewarding role combines direct patient interaction with a wide range of clinical, technical, and administrative responsibilities. You will be helping individuals achieve better health outcomes through tailored support, informed interventions, and collaborative care planning.
Within a multi-disciplinary team, you will form positive, respectful relationships with service users, families and Carers
Applying evidence-based practices to assess needs, implement care plans, monitor progress, and escalate concerns. You will also promote health and wellbeing, safeguarding individuals, and ensuring a safe and inclusive environment for all.
You will contribute to service development and improvement through audits, digital innovation, and research initiatives. You will also support colleagues and students, being a role model and advocate for learning and professional growth.
