Job overview

We are looking for an ACP to join our team on the physical health wards at Shaftsbury Hospital. You will be based on Shaston/Ashmore ward which is a 15 bedded ward. We care for patients who have been stepped down from an acute trust or require step up from the community, including end of life care.

The complexity of our patients needs require a practitioner with experience in frailty, end of life care and a portfolio of knowledge covering conditions like Parkinson’s, Heart failure, Diabetes, and Alzheimer's with an extensive pharmacology knowledge, and the ability to act autonomously.

You will conduct  assessments, physical examinations and interpret diagnostics, implement evidence-based care and provide expert clinical advice to colleagues, patients, and families. You will monitor patient progress, evaluate effectiveness of interventions and adjust care accordingly as well as  participating in clinical audits, research and quality improvement initiatives to enhance service delivery.

There is plenty of scope for collaborative working with the ACP network and training and clinical supervision from our Professional Lead, Medical Lead and Clinical Director. The team at Alderney Hospital are skilled and enthusiastic and are looking for an ACP who is knowledgeable, approachable, has strong leadership qualities and is a good communicator with excellent interpersonal skills.

Main duties of the job

To provide highly specialist high-quality person-centred care using advanced clinical knowledge that is evidence based.

As part of a multi-professional team, the post holder will practice autonomously, whilst being accountable and self-directed in line with the relevant code of professional conduct.

To plan and manage complete episodes of care; undertake independent comprehensive assessment and management of service users to incorporate pharmacological considerations; making complex clinical decisions regarding service user management and clinical outcomes.

To provide expert clinical advice, leadership and support ensuring the needs of the service are met by exercising and demonstrating high levels of clinical judgement, critical analysis and advanced decision-making skills.

To lead as a driver for change, to monitor and improve standards through supervision, evidence-based practice, clinical audit, research, and education. To promote and demonstrate best practice by integrating evidence into practice.

To work strategically, contributing to the planning and development of integrated services.

The post holder will be accountable for the direct delivery of autonomous service user care.
