Job overview

The post holder will practice as an Advanced Clinical Practitioner within the Frailty Service  providing expert care and management to patients with a variety of frailty related conditions across the inpatient and urgent outpatient setting based in our Virtual Ward. HDFT ACPs are embedded as an integral part of the Medical Team. They are expected to make complex, autonomous decisions utilising expert practice and knowledge and evidence-based medicine. They work as part of a multi-disciplinary team ensuring high quality patient care.

The post holder will contribute to a safe, patient focused service. This includes assessment, diagnosis, treatment, referral, and discharge of patients with a variety of presentations and diagnoses.

Support and designated time will be given to contribute to service improvement, research, and education.

Main duties of the job

To deliver and participate in the clinical care of patients within the Frailty Service.

Prioritize health needs and intervene appropriately.

Provide advanced and comprehensive assessment based on a specialist body of knowledge.

Direct responsibility for management of allocated patients. Including appropriate assessment, examination, investigations, diagnosis and treatment resulting in safe management and appropriate referral or discharge.

Order and interpret investigations, laboratory tests, x-rays and act on results as appropriate.

Be an Independent Prescriber and initiate drug therapy/medications as per legal requirements for the practitioner’s professional body, national and local guidelines.

To be professionally and legally accountable for all work undertaken. Practice at an advanced level of professional autonomy and accountability that is within the Trusts policies and professional code of conduct (NMC, HCPC).

Ensure that clinical documentation is comprehensive, accurate and contemporaneous.

Ability to adapt specialist clinical knowledge and skills to different clinical settings and influence service delivery and patient care.
