Job overview

As a Case Manager, the post holder will work as part of a dynamic team in delivering an effective service to ensure safe and effective care pathways are in place for all service users from their hub in specialised mental health care settings, within the NHS and Independent sectors.

To provide clinical support to the specialist commissioners in relation to the defined population, this will include projected mapping of future service use, identification of users for future service provision, planning in terms of future service needs.

The role supports the business in driving transformation as well as value for money in planning, commissioning and service the role is designed to build a combination of subject matter expertise and technical skills to develop a strong service delivery.

Please note for this role you will need the ability to travel independently and in a timely manner across Cambridgeshire and Peterborough to meet strict time deadlines.

  • To provide clinical assurance of the quality of the care provided by specialised services  within the Hub.  • Supporting the team in the assurance of specialised services compliance to applicable specification and guidance. • The post-holder will provide the link with Clinical Commissioning Groups and their commissioned services within the Hub to ensure there is a co-ordinated approach to care and discharge planning, which results in robust and effective care packages. • Managing individual patient pathways to ensure high quality services and care planning outcomes are achieved • To take action in respect of Safeguarding Policies and Procedures as required
  • To ensure that clinical leadership is central to the delivery of all NHS Commissioning Board activities • To work collaboratively across the NHS England matrix, including integrating the National Director’s portfolio with the Domain Leads. • To participate in clinical/professional supervision Enabling patient and public involvement • To act as a champion for patients and their interests and involve the public and service users.  • To ensure all public and patient contact with the office is of the highest professional standard • Ensure services have mechanisms in place for patient and carer involvement in their care plans
