An opportunity has arisen for an experienced Admiral Nurse or other dementia specialist nurse within the Bradford District Care Trust team of Admiral Nurses, part of the award-winning Proactive Care Team (PACT).

An Admiral Nurse is a senior nursing position specializing in dementia care, relationship-centred support to individuals with dementia and their families, including complex assessments, clinical interventions, education for other healthcare professionals, and advocacy for quality dementia care within a designated area, all while adhering to the Admiral Nurse competency framework.

Main duties of the job

The primary focus of the Admiral Nurse role is to deliver person –centred and relationship- centred dementia care within central Bradford to improve the experience of people living with dementia, their carers and families.

The post holder will:

  • Deliver evidence based, culturally appropriate dementia care and support improvements in the experience of people living with dementia, their carers and families, applying the Admiral Nurse Competency Framework at a specialist level.
  • Work with families to provide support, expert guidance and practical solutions
  • Work closely with other services and organisations to co-ordinate care for people living with dementia and support for their families including planning for future care, access to care for co-morbidities and access to specialist palliative care.
  • Provide support/education to colleagues, other health and social care professionals/providers in dementia care.
