Job summary

Main duties of the job

  • To ensure that patients with clinical deterioration have their needs prioritised with appropriate assessment, intervention and on-going management. To ensure that patients at risk are reviewed regularly and appropriate intervention initiated in a timely manner.
  • The post holder will assist with the delivery of education through enhancing staff confidence, competence and experience of ward staff through sharing of skills.
  • The service will utilise physiological track and trigger tools to identify patients whose condition is deteriorating via the Electronic patient record.
  • They will assist/support audit projects and update guidelines/policies within the Acute Intervention Nurse remit
  • Assist / Support ward staff with care of the dying patients, complex DNACPR decisions and ensure effective communication with the patient and their families.
  • To assist the anaesthetic team to stabilise critically ill patients throughout the hospital.
  • To be an active member of emergency response teams within the hospital, taking a lead role in the Medical Emergency Team.
  • Capable of acting as team leader in the Cardiac Arrest Team when required.
  • Active member of the Paediatric Emergency Team

We provide hospital services from two acute sites - Darlington Memorial Hospital and University Hospital of North Durham. We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes.
