Job overview

We are looking for a motivated and compassionate Advanced Clinical Practitioner (ACP) to join our Urgent Community Response (UCR) and Hospital Intervention Team (HIT).

This is an exciting, autonomous role where you will work across ED, acute wards, and patients’ homes, supporting admission avoidance, early discharge, and integrated care pathways including Virtual Ward (Hospital@Home).

What you will do :

  • Deliver advanced assessment, diagnosis and treatment
  • Identify patients for admission avoidance and early supported discharge
  • Act as a senior clinical decision-maker to improve patient flow
  • Work collaboratively across acute, community, and partner services
  • Lead and contribute to pathway development and service improvement
  • Provide clinical leadership, teaching and support to the MDT

About you : You will be:

  • A registered professional (NMC/HCPC)
  • An ACP (or working towards qualification)
  • Experienced in acute, urgent, frailty or community care
  • A confident decision-maker with strong clinical judgement
  • Able to work independently and across organisational boundaries
  • A skilled communicator who can lead, influence and inspire others

Main duties of the job

  • Provide and deliver advanced clinical assessment, diagnosis,               management and treatment for patients presenting in ED, acute            and gateway wards, and in the community
  • Lead on identifying patients suitable for admission avoidance, early supported discharge or step‑down community pathways.
  • Act as a senior clinical decision‑maker to support safe, timely discharge and prevent unnecessary hospital stays.
  • Work collaboratively with UCR, Virtual wards, HIT, ED clinicians, ward teams, therapy services, social care and VCSE partners
  • Support pathway development and service improvement to enhance hospital‑to‑community integration.

Clinical Environment

The role spans: Emergency Department (ED), Acute and gateway wards, Patients’ homes and community settings.

Person Specification (Essential)

  • Registered healthcare professional (NMC/HCPC) with ACP qualification or working towards recognised ACP qualification. (Would consider a trainee position for the right candidate at band 7 - to progress to 8a once qualified)
  • Significant experience working at an advanced level within acute, frailty, urgent or community services
  • Strong clinical assessment and decision‑making skills across complex, undifferentiated presentations.
  • Demonstrable experience of cross‑system working and pathway development
  • Ability to work autonomously and confidently across organisational boundaries.
  • Excellent communication, leadership and influencing skills.
