An exciting opportunity has arisen to develop and deliver prehabilitation and rehabilitation pathways across Barnsley and help thousands of patients across South Yorkshire.

Active Together is a prehabilitation and rehabilitation service supporting people following a cancer diagnosis with physical activity, nutrition, and wellbeing guidance. The service has been designed by Sheffield Hallam University who work in close collaboration with NHS teams to support the delivery and evaluation of the service.

The Active Together service launched in Sheffield in 2022 and is currently expanding to new sites across South Yorkshire in Barnsley, Rotherham and Doncaster. Active Together South Yorkshire will provide equitable, multi-modal and personalised prehabilitation and rehabilitation support that meets the needs of patients across the region and generates evidence for a commissioned service.

Barnsley Hospital Foundation Trust and Barnsley Premier Leisure are working in partnership to deliver this programme to the population of Barnsley.

Active Together is funded by Yorkshire Cancer Research, Yorkshires independent cancer charity, working so more people can live longer, healthier lives, free from cancer.

To have lead responsibility for the development and delivery of the Dietetic Active Together service.

Support service redesign and development of new cancer services taking into account the ambitions of national NHS long-term conditions and cancer strategies

Demonstrate the impact of the defined services on the local healthcare system and people affected by cancer.

To provide expert clinical practice to a complex caseload of people affected by cancer within the defined services.

To benchmark and research best practice to develop and maintain professional therapy services which would be of benefit to patients on defined cancer pathways.
