We are looking to recruit a motivated and dynamic Community Learning Disability Nurse/ Community Mental Health Nurse for a fixed term period to operate as an autonomous practitioner providing person centred health checks for people with a learning disability across the region. The post holder will work in partnership with patients, carers and other care providers, promoting the principles of social inclusion.
This is a fixed term position for 9 months, from start date
The post holder will be responsible for the delivery of annual health checks and will support interventions to meet the individual’s assessed health care needs. The Nurse will have knowledge and awareness of issues impacting on adults with learning disabilities, and will have the skills, experience and knowledge to direct appropriate action to support patients.The post holder will work closely with primary care across the region, working pro-actively and creatively to contribute to the health improvement of people with a learning disability. Through health promotion activity, developing relationships with partners, whilst including undertaking the health screening process, the aim is for all people with a learning disability in Dumfries & Galloway to have access to yearly health checks and required health care.
To post holder will work within primary care settings but will work in collaboration with the Community Learning Disability team and will support the Professional Lead Nurse to assist with strategic aims, research and development of health check service and nurse education within the speciality.
The post will include some responsibility for the implementation and monitoring of a quality assurance system.

As from 1st April 2026, the Agenda for Change full-time working week will be reducing from 37 to 36 hours per week. Part time hours will be reduced pro-rata. However, there will be a corresponding increase in the hourly rate so that pay will not be affected.
