Job summary

Main duties of the job

You will work as part of the Neonatal Outreach Team, demonstrating flexibility, reliability and strong team working.

Provide high quality neonatal care to babies and families prior to and following discharge from the Neonatal Unit.

Maintain excellent communication within the team to ensure safe and coordinated care.

Undertake planned and responsive home visits and telephone follow up.

Support parents/carers with feeding, growth, development and ongoing neonatal care needs

Contribute to effective discharge planning and continuity of care.

Act as a key link between the Neonatal Unit, community services and primary care.

Work collaboratively with neonatal consultants, ANNPs, ward staff, health visitors and the wider MDT.

Deliver education, advice and emotional support to parents/carers.

Maintain accurate, timely and professional documentation in line with Trust policy.

Participate in audit, service evaluation and quality improvement.

Positively contribute to team dynamics, supporting colleagues and service resilience.

Adapt working practices to support service continuity, including prioritisation and time management.

Act as a role model and support the supervision and development of junior staff as appropriate.
