As a Patient Pathway Coordinator you will join a dynamic team that works with clinical and non-clinical teams to support us in ensuring that our patients receive timely treatment. You will work to support the effective management of elective pathways, ensuring services are delivered efficiently and effectively, meeting the national elective targets.
Main duties of the job
- The post holder will join a team of Patient Pathway Coordinators who assist the operational teams in the validation of information to support elective waiting times across the Trust.
- To be proficient in the use of the Trust system (ICare) to validate the patients' Referral to Treatment (RTT) pathway.
- To work with operational staff to ensure that accurate and timely data is recorded in accordance with national and local guidance.
- To work with operational teams ensuring that patients' pathways are being appropriately managed and escalating issues to prevent delays to patient care.
- Required to communicate complex information and processes to relevant staff.
- To undertake other duties as requested by your team leader which are appropriate to the role to ensure the smooth running of the team.

- The post holder will work closely with Directorate teams across all sites to provide an administrative service to support the elective pathway.
- To liaise with the other departments to ensure all patients have a booked first appointment, investigation and procedure.
- Report to the Management team any deviations from process.
- Undertake validation and corrections to RTT pathways ensuring the pathway is kept up to date.
- Establish good working relationships in order to improve the coordination of the patient journey.
- To provide support to elective projects as required.
- To support all services and cross cover the other members of the team during periods of annual leave or absence.
