### Job summary

Are you passionate about helping people improve their health, wellbeing, and employment outcomes? We are looking for a motivated and compassionate WorkWell Coordinator to join our WorkWell for Health service.

In this rewarding role, you will support individuals whose health is impacting their ability to stay in work, return to work, or move closer to employment. Managing a diverse caseload, you will undertake holistic assessments, develop personalised WorkWell Plans, and coordinate access to a range of health, wellbeing, employment, and community support services.

Working closely with healthcare professionals, local authorities, employers, and voluntary sector partners, you will act as a trusted single point of contact, empowering people to overcome barriers, achieve their goals, and improve their overall quality of life.

This is an exciting opportunity for someone who is organised, empathetic, and committed to delivering person-centred support that makes a real difference to individuals and communities across Hertfordshire.

### Main duties of the job

Manage a caseload of participants throughout the WorkWell pathway, ensuring timely, personalised and person-centred support.

Undertake holistic work and health assessments to identify individual strengths, barriers and support needs, using recognised assessment tools where appropriate.

Co-produce personalised Back to Work or Thrive at Work Plans with participants, setting realistic goals, agreed actions and measurable outcomes.

Build trusting relationships using coaching, motivational interviewing and behaviour change techniques to promote confidence, resilience and self-management.

Empower participants to build the knowledge, confidence and skills required to manage their health and wellbeing and achieve sustainable work-related outcomes.

Coordinate timely referrals and signposting to appropriate health, wellbeing, employment and community support services, working collaboratively with partner organisations.

Monitor participant progress through regular reviews, adapting support plans to reflect changing needs and circumstances.

Liaise with employers, occupational health providers and partner organisations to support individual participants and facilitate sustainable employment outcomes where appropriate.

### Knowledge and Experience

**Essential**

- Experience of managing a caseload and delivering person-centred suppor
- Experience of undertaking holistic assessments and developing personalised support plans.
- Experience of supporting people with health-related barriers, including physical health, mental health or wider social issues.
- Experience of working collaboratively with health, social care, voluntary or community organisations.
- Experience of coordinating referrals and supporting individuals to access appropriate services.
- Excellent communication and interpersonal skills, with the ability to build rapport and motivate individuals.
- Ability to manage competing priorities and work independently.
- Good IT skills and experience of maintaining accurate electronic records
- Knowledge of safeguarding, confidentiality and information governance.
- Understanding of the wider determinants of health and the impact of health on employment.

**Desirable**

- Experience working within Primary Care, community health, social prescribing, health coaching, employment support or vocational rehabilitation
- Knowledge of the WorkWell programme or similar health and work initiatives
- Training in Motivational Interviewing, Health Coaching or behaviour change approaches.
- Experience of partnership working across NHS, local authority, VCSE or employment services
- Experience of collecting outcome measures and reporting performance data
- Knowledge of local health, wellbeing and community support services across Hertfordshire

### Education and Qualifications

**Essential**

- Good standard of education, including GCSE (or equivalent) in English and Mathematics.
- Relevant vocational qualification or equivalent experience in health, social care, employment support, wellbeing, public health or a related field

**Desirable**

- Level 3 or Level 4 qualification in Health & Social Care, Advice & Guidance, Coaching, Community Development, Public Health or a related discipline.
- Health Coaching, Motivational Interviewing or Behaviour Change training
- Safeguarding Adults and Children training
- Mental Health First Aid or equivalent wellbeing qualification.
