We are looking for a Healthcare Assistant to join our Community Team based in West Dorset, helping to deliver exceptional care to patients with life limiting illnesses. Working alongside our Community Nurses and wider multidisciplinary team, you will support patients with their personal care needs, promote dignity and independence, and provide practical and emotional support to both patients and their loved ones. This role is ideal for someone who is compassionate, motivated, and committed to providing outstanding person-centred care. Please note, this is a full-time position of 37.5 hours a week with a weekend working rota approximately 1 in 6. The post holder will work collaboratively with clinical colleagues and external agencies to provide an agile and responsive service based on needs identified by the Specialist Hospice at Home team. Provide a reassurance to patients and/ or families, reducing the likelihood of hospital admission.
