Alderney Ward Therapist

Are you an enthusiastic Occupational Therapist looking to broaden your practice within our dedicated specialist team? Join our ward team.

As a ward Occupational Therapist based at Alderney hospital you will bring your clinical and inspiring leadership expertise to the patient journey, you will be assessing and leading on the discharge process to get people back into the community and supporting flow from the acute hospitals. You will be joining a supportive, multidisciplinary team providing highly specialist advice and programmes of care to other professionals, to ensure high quality person – centred care and interventions. There is an expectation to work across sights as needed. With your effective communication, professionalism and innovation, care delivery to our patients will be of the highest standard. ### Main duties of the job

Providing OT based assessment and discharge planning for a range of physical presentations such as stroke and neurology, orthopaedics, frailty, falls, cognitive impairment and many more in a ward setting.

Knowledge and experience in moving and handling, experience of using a range of equipment, experience of carrying out cognitive assessments, skills to holistically assess and formulate a patient centred rehab program, knowledge and experience of working with the older person, particularly with frailty.

Ability to recognise and manage a deteriorating patient, problem solving and crisis management of an acute situation, skills in carrying out a basic physical assessment including basic observations and recognising when it is appropriate to escalate.

Carry out rehab and assessment on the rehab ward, complete home visits/dc visits as needed, support the therapy assistants, students and the ward as a whole.

Demonstrable skills in communication, organisation, caseload management, autonomous working and supervision and mentorship of both trained and junior colleagues.

Problem solving and innovation to inform service development and improve quality standards.

A large part of this role is overseeing and planning complex discharges, working closely with the MDT and ensuring patient flow through the ward from the acute hospitals.

Employment in this post requires an Enhanced/Standard Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of.

- Experience at Practitioner Band 5 level
- Experience of specialist working
- Recent previous experience within a comparable role
- Experience of managing change
- Demonstrable knowledge of assessment and therapeutic interventions in area of specialism
- Experience of developing specialist programmes of care for an individual or groups of patients/clients and of providing highly specialist advice
- Able to demonstrate specialist clinical reasoning skills to assimilate information in order to make a clinical judgement regarding diagnosis and intervention.
- Ability to prioritise and organise workload effectively

- Experience of planning and organising complex activities, e.g. organise own time and that of junior staff and learners, planning off duty rotas and undertaking discharge planning involving co-ordination with other agencies.

- Willingness to advance own clinical knowledge, skill and competence based on current evidence

### KNOWLEDGE, SKILLS AND TRAINING

**Essential**

- Registered practitioner to degree/diploma level supplemented by post registration diploma level specialist training and/or short courses or demonstrable extensive experience in the relevant specialty
- Membership of the relevant Professional Body
- Learning and Assessing in Practice Qualification or equivalent practice assessors training
- Evidence of recent professional development in an up to date portfolio

**Desirable**

- Knowledge and understanding of Trust Strategy relevant to role

### INFORMATION TECHNOLOGY/RESOURCES

**Essential**

- Able to analyse data and produce reports using Microsoft Excel and Word
- Experience of using electronic patient / service user record systems

### MANAGERIAL/SUPERVISORY EXPERIENCE

**Essential**

- Experience of providing clinical supervision and mentoring to junior staff
- Experience of devising and delivering training
