You will work within the Virtual Ward Team and in collaboration with system partners such as City Care, as part of a supportive multi-disciplinary clinical team with a dedicated focus of delivering excellent patient centred care and innovation. You must be able to work flexibly over 7 days including weekend and bank holidays. Shifts are 8am -8pm You will be providing care to patients in a community setting, who require high quality holistic evidence-based care that is responsive, effective, and efficient in line with the virtual ward model.
