### Job overview

This role is for 9 months fixed Term or a DVH secondment. The post holder will support patients and their family and friends with urology cancers. They will support during the cancer journey from referral to diagnosis, through treatment and follow-up with living with and beyond cancer, covering elements of the recovery package including personalised care and support metrics..

The post holder will provide evidence-based patient information sensitive to individual need. This will involve psychological and informational support respecting at all times patients’ diverse cultural back grounds, with courtesy, dignity and privacy.

They will liaise and effectively communicate with members of the multi-disciplinary team and allied health care professionals to provide co-ordinated high-quality patient-centred care, act autonomous applying specialist expertise to all facets of care of patients, and develop and implement nurse-led clinics.

### Main duties of the job

Provide expert clinical, holistic and Psychological care and support to our Cancer patients

Co-ordination for patients diagnosed with suspected or confirmed urological cancers, including prostate, bladder, kidney, testicular and penile cancers. In line with national cancer standards 28-day FDS and 62 day target.

Provide expert clinical assessment, advice, symptom management to patients at home, clinics and wards.

Support patients & families with information, emotional support, and treatment decision making. For patients at home, clinic and wards.

Co-ordinate investigations, treatment plans, follow up care Across the multidisciplinary teams (MDT)

Leading or contributing in nurse led clinics, treatment reviews, and survivorship care.

Support patients having treatment – Surgery, radiotherapy, immunotherapy chemotherapy, hormone therapy, or active surveillance with managing side effects and complications and refer onto other services when appropriate.

Completing and reviewing Holistic needs assessments and developing personalised care and support plans as per national cancer plan.

You must have appropriate UK professional registration.

### Experience

**Essential**

- Minimum 3 years in acute setting, preferably 2 years as Senior Nurse within specialty
- Experience of urology cancer care management and in-depth knowledge of the disease processes

**Desirable**

- Experience of working within a Cancer Centre
- Experience of audit/research
- Experience of resource management
- Experience of gathering and analysing detailed information
