An exciting opportunity has arisen to join the Shropshire, Telford & Wrekin Urgent Community Response & Virtual Wards service as a Staff Nurse. The post will form part of a seven day service based within one of the Trust’s locality teams. The service provides patients who are experiencing an acute change in their health, with a safe and effective alternative to hospital admission, supported by a mixture of face to face visits and digital technology.

We are looking for a Registered Nurse who shares our passion for providing excellent quality of care and can help deliver a recommended and agreed treatment/care plan to service users in their own home. The post holder will have full access to the Multidisciplinary Team including consultant, GP, Pharmacist, Nurses, Therapists, and Health Care Support Workers.

### Main duties of the job

As a service which is at the forefront of the NHS Long Term Plan agenda to develop Urgent Community Response services and improve our local system responsiveness to managing community health crisis response services, the role on offer represents an ideal opportunity for a Registered Staff Nurse to join the established Multidisciplinary Team and continue to help shape our future.

Working alongside medics, social workers, therapists, and in partnership with GPs, Shropdoc, 111 WMAS, CCC and other healthcare professionals your role will be to ensure the continuation of a seamless service to avoid unnecessary hospital admissions.

The successful applicant will require the appropriate skills and knowledge outlined in the person specification, and be registered with the NMC, as well as share the Trusts vision to provide assessment and delivery of comprehensive, holistic, evidence-based nursing and care support for the management of acutely unwell patients referred to the Shropshire UCR.

- Pre or post registration experience in general nursing practice
- Previous NHS experience
- Experience of working in a multi disciplinary / multi-agency health / social care environment
- Experience and knowledge of IT basic administration systems including Microsoft Office Suite (Word, Excel etc).

### Knowledge and Skills

- Ability to demonstrate a level of English language and grammar commensurate with writing in service user case notes
- Ability to carry out comprehensive holistic nursing assessment
- Awareness of current community nursing skills
