The post will form part of a seven day service based within the Bridgnorth locality team. The service provides patients who are experiencing an acute change in their health, with a safe and effective alternative to hospital admission, supported by a mixture of face to face visits and digital technology. We are looking for a Registered Nurse who shares our passion for providing excellent quality of care and can help deliver a recommended and agreed treatment/care plan to service users in their own home. The post holder will have full access to the Multidisciplinary Team including consultant, GP, Pharmacist, Nurses, Therapists, and Health Care Support Workers. Working alongside medics, social workers, therapists, and in partnership with GPs, Shropdoc, 111 WMAS, CCC and other healthcare professionals your role will be to ensure the continuation of a seamless service to avoid unnecessary hospital admissions.
