This role plays a vital part in reducing unnecessary hospital stays and supporting smooth transfers of care. You'll work within a multidisciplinary team alongside health and social care partners. Using your clinical skills and judgement, you'll assess needs, agree next steps with patients and carers, and help plan the right level of support.
