We are seeking seven enthusiastic, experienced and highly motivated Trust Grade Registrars to join our Anaesthesia and Critical Care departments at London North West University Healthcare NHS Trust (LNWHT). These are exciting new posts created by the merger of our previously separate Ealing Hospital and Northwick Park/Central Middlesex rotas into a single unified, three-site rota. This change represents a significant expansion in clinical breadth for post holders, and a unique opportunity to work across one of the largest and most diverse Intensive Care and Anaesthesia departments in North West London.

The posts are primarily designed for doctors with established experience in Intensive Care Medicine who are capable of functioning as the senior resident registrar within a consultant-led critical care service. Successful applicants will participate in resident on-call rotas covering Intensive Care and Enhanced Care services across all three hospital sites. During out-of-hours periods, post holders will frequently act as the most senior resident doctor within the Critical Care service, supported at all times by an on-call Intensive Care Consultant.

The post holder will be expected to participate fully in the provision of a comprehensive Intensive Care and Anaesthetic service across all three hospital sites, accountable to the Clinical Directors?.

Critical Care Responsibilities

- Participate in the resident on-call rota covering Intensive Care and Enhanced Care services across Northwick Park, Ealing and Central Middlesex Hospitals.
- Function as the senior resident registrar within the Critical Care service, supported at all times by an on-call Intensive Care Consultant.
- Assess, investigate and initiate management of critically ill patients presenting with respiratory, cardiovascular, neurological, renal, metabolic and infectious pathologies.

Anaesthetic Responsibilities

- Provide anaesthesia and analgesia for patients undergoing elective and daytime CEPOD surgery across all Trust sites, exclusively under the direct, immediate supervision of an allocated Anaesthetic Consultant.

Anaesthetic Responsibilities

- Provide anaesthesia and analgesia for patients undergoing elective and daytime CEPOD surgery across all Trust sites, exclusively under the direct, immediate supervision of an allocated Anaesthetic Consultant.
- Post holders will not hold an anaesthetic on-call bleep. Out-of-hours commitments are entirely within Intensive Care and Enhanced Care services.
- Undertake pre-operative assessments and provide post-operative care in the recovery room.
- Advise surgical and pre-operative assessment teams on the preparation of patients for surgery.
- planned daytime rota commitments together with eventual progression to oncall work on the maternity unit as part of the Obstetric Anaesthetic service.
- Provide opportunities to help develop anaesthetic competencies appropriate to previous experience and career aspirations, supported by the allocated Educational Supervisor.

Critical Care Responsibilities

- Participate in the resident on-call rota covering Intensive Care and Enhanced Care services across Northwick Park, Ealing and Central Middlesex Hospitals.
- Function as the senior resident registrar within the Critical Care service, supported at all times by an on-call Intensive Care Consultant.
- Assess, investigate and initiate management of critically ill patients presenting with respiratory, cardiovascular, neurological, renal, metabolic and infectious pathologies.
- Lead the resident medical response to deteriorating patients referred from wards, emergency departments, theatres and recovery areas.
- Participate in consultant-led ward rounds and contribute to the formulation of management plans. There will be opportunity to lead ward rounds in an acting-up or upskilling capacity.

### Qualifications

- Advanced Life Support (ALS) provider certificate or equivalent

**Desirable**

- Final FRCA, FFICM, EDIC or CICM

### Personal Skills

**Essential**

- - Appropriate knowledge and practical skills to recognise and manage high-risk patients in the ICU and perioperative environment
- Ability to prioritise workload and make sound clinical decisions in a busy acute environment

- - Point-of-care ultrasound or critical care echocardiography skills
- IT skills including use of electronic health records
- Awareness of current developments in Intensive Care Medicine and Anaesthesia

### Management, Audit and Research

**Essential**

- Ability to manage own workload/time and train medical staff.
- Participation in clinical audit Understanding of audit principles and commitment to its use as a tool for raising and maintaining standards.
- Able to appraise scientific publications and perform literature searches.

**Desirable**

- Evidence of ability to lead a team.
- Ability to develop and lead on audit project.
- Evidence of research undertaken, presentations at meetings, publications in peer reviewed journals.

### CLINICAL EXPERIENCE, KNOWLEDGE AND EFFECTIVENESS

**Essential**

- -
- Minimum of 4 years postgraduate clinical experience
- Minimum of 2 years experience working in an Intensive Care Unit, with the clinical capability to function as the senior resident registrar in a consultant-led ICU, in conjunction airway management experience set out below, which is required to carry the outreach and ICU resident bleeps
- Experience managing critically ill patients with respiratory failure, septic shock, multi-organ dysfunction and major perioperative complications
- Experience managing patients requiring invasive and non-invasive ventilation
- Experience managing patients with circulatory failure requiring vasopressor or inotropic support, including the ability to recognise, initiate and titrate vasopressor and inotropic therapy
- Experience of advanced airway management including tracheal intubation, including the ability to independently lead and manage induction and intubation of a critically unwell patient
- Experience of arterial and central venous catheter insertion
- Experience of intra-hospital and inter-hospital transfer of critically ill patients
- Experience managing tracheostomised patients
- Experience managing patients receiving renal replacement therapy
- Experience supervising junior medical staff

**Desirable**

- Further experience in anaesthetics or intensive care or acute medical specialties Awareness of specialty developments. IT skills including use of databases, spread sheets, Word, PowerPoint etc. and willingness and aptitude to learn how to use CERNER (electronic patient record system).
