This pivotal role supports system flow, admission avoidance and timely discharge pathways through the delivery of high-quality, patient-centred triage. You will act as the point of contact for hospital discharge referrals and urgent community referrals, you will use your clinical judgement to direct individuals to the most appropriate Intermediate Care pathway to ensure people receive the right care, first time. Working within a virtual MDT with system partners, you will apply sound clinical judgement to direct individuals to the most appropriate Intermediate Care pathway.
