The post holder will lead the development and delivery of nurse-led services, working collaboratively with the multidisciplinary team to improve patient outcomes, promote best practice, and support service development within renal transplantation. ### Main duties of the job Key Responsibilities - Lead and develop a nurse-led transplant service, providing specialist care and support to renal transplant patients throughout their transplant journey. - Assess, plan, implement and evaluate complex care for patients within a variety of clinical settings, ensuring care is evidence-based and individually tailored. - Manage and coordinate a defined caseload of transplant patients, facilitating continuity of care and effective follow-up. - Lead the development, implementation and review of transplant care pathways, protocols, guidelines and clinical standards. - Provide highly specialised clinical advice and support to patients, carers, nursing staff and the wider multidisciplinary team. - Deliver nurse-led outpatient clinics in partnership with medical colleagues, contributing to the ongoing monitoring and management of transplant recipients. - Act as an expert practitioner and professional role model, promoting clinical excellence and supporting the development of others. - Provide specialist education, training and mentorship to healthcare professionals, students, patients and carers. - Integrate research, audit, education and practice development into clinical service delivery to enhance patient outcomes and experience. - Lead and participate in clinical audit, quality improvement and service evaluation activities within the transplant service. ### Detailed job description and main responsibilities Establish nurse led clinics, providing expertise to patient, staff and carers in relation to Transplantation. Take responsibility for the co-ordination and management of a patient caseload linked to a nurse led clinic, demonstrating high levels of planning and organisational skills. Work as an expert practitioner demonstrating advanced skills in chronic illness management within the speciality of outpatient transplant care and the co-ordination of patient care. Devise and implement care pathways and protocols following unit guidelines to enhance patient care. In conjunction with the transplant team develop and implement a transplant strategy with the aim of improving patient care and promoting the further development of nursing within the speciality taking into account audit, patient and staff feedback and Trust policies, and ensure appropriate dissemination. Be responsible for a caseload of patients identified for nurse led follow up clinics by consultant teams, including physical clinical assessments where appropriate. Select and order laboratory/screening tests for patients within unit protocols according to best practice and to individual need. Review these and other medical test results interpreting results and deciding upon appropriate nurse led action with referral to medical team if required. Be able to respond knowledgeably and appropriately to normal and abnormal biomedical results, and communicate with medical/surgical staff, and keep patients informed of changes, ensuring the renal database is updated Monitor blood results and medication doses for identified patients within designated protocols. Daily review of blood results/investigations of transplant patients attending outpatient clinics. Liaise with medical staff and GP’s regarding management according to unit protocols. Review and manage patient care pathways according to unit protocols. Develop and utilise a thorough understanding of relevant unit policies, procedures and protocols in order to both review and manage care pathways and advise colleagues ### Applicant requirements You must have appropriate UK professional registration.
